Excessive Noise Phase 90 (video demo)

Any idea on how to identify the source of this noise?
It only occurs when the phase is on, no issues when it's off.
I replaced the switch, cleaned and resoldered the jacks and pot. Resoldered a few pieces on the board including the battery terminals.
Toggling the pot has no effect.
I bought an original MXR Phase 90 and used it for one gig with no problems and now this noise problem has surfaced.
